La Società Italiana di Malattie Infettive e Tropicali – SIMIT avvia un importante progetto per impegnarsi nella lotta contro i microrganismi multiresistenti agli antibiotici, fenomeno in crescita in tutta Europa, con l’Italia che è tra i Paesi con le peggiori performance.
IL PROGETTO DELLA SIMIT CONTRO I MICROBI MULTIRESISTENTI – Il progetto Resistimit consiste in una piattaforma clinica che mette in rete diversi centri di malattie infettive che lavoreranno insieme per creare un registro dinamico sulle infezioni multiresistenti in Italia. A far partire l’iniziativa sono dieci centri pilota, dislocati in tutte le aree del Paese: Roma con Spallanzani, Tor Vergata e Umberto I, Napoli con Cotugno e Federico II, Bari, Foggia, Palermo, Pisa, Varese, Modena, Perugia, Padova. Non appena giungerà l’approvazione dei comitati etici, il progetto si estenderà ai centri di malattie infettive che vorranno partecipare. Infettivologi di diverse aree d’Italia, in rappresentanza di questo nucleo fondativo, si riuniscono a Pisa, presso l’Hotel San Ranieri, il 14-15 dicembre, con il convegno “La resistenza agli antimicrobici nella real-life” finalizzato all’analisi del fenomeno e alla stesura di un protocollo di operatività. “L’obiettivo è quello di creare una struttura che permetta di ottenere un registro degli organismi multiresistenti nelle varie regioni italiane tramite applicativi informatici idonei – spiega il Prof. Marco Falcone, Segretario SIMIT e Professore Ordinario di Malattie Infettive all’Università di Pisa – Oggetto di questo studio saranno batteri, funghi, virus e ogni altro microrganismo resistenti ai farmaci antimicrobici. Il registro sarà funzionale a diversi scopi: da un lato, consentirà di monitorare il fenomeno dell’antibiotico-resistenza; dall’altro permetterà di indagare caratteristiche e meccanismi di acquisizione delle infezioni causate da questi microrganismi nelle persone più colpite; inoltre, sarà la base anche per pianificare ulteriori approfondimenti sui nuovi farmaci antimicrobici. Abbiamo a disposizione molecole attive molto interessanti, ma si deve adottare un uso attento, che però non significa una marginalizzazione degli antibiotici, che restano preziosi salvavita. I nostri centri clinici devono fornire ai decisori, compresa AIFA, un supporto tecnico-scientifico basato su dati di real-life per dimostrare efficacia e sicurezza dell’uso degli antibiotici nel nostro Paese”.
L’IMPEGNO DEGLI INFETTIVOLOGI DELLA SIMIT – “Il progetto Resistimit è curato da un board di giovani infettivologi con una profonda esperienza sia clinica che di ricerca – sottolinea il Prof. Claudio Mastroianni, Presidente SIMIT e coordinatore del progetto insieme al Prof. Falcone – Questa iniziativa rientra nelle strategie della nostra società scientifica per coinvolgere le nuove generazioni di specialisti nella costruzione di un network per la raccolta dei dati e delle informazioni contro un fenomeno quale quello dei microbi multiresistenti, caratterizzato da numeri allarmanti e dall’assenza di adeguate contromisure. In Italia, infatti, manca una reportistica adeguata: sappiamo che esiste questo problema, ma non ne conosciamo le dimensioni, non abbiamo certezze su quali siano le infezioni più gravi e quelle più difficili da trattare. Con questa iniziativa si offrirà alla comunità scientifica un prezioso strumento per analizzare nel dettaglio tutte le sfaccettature di questa problematica, utile anche per istituzioni e altri enti che volessero ottenere informazioni aggiornate sulle infezioni provocate da microrganismi multiresistenti”.
Tra le iniziative già avviate in questi mesi nella lotta ai batteri multiresistenti vi è anche la piattaforma messa a punto presso il Policlinico di Tor Vergata, un software in cui vengono inseriti tutti i fattori utili per diminuire la resistenza dei germi e per capire quale fattore abbia provocato l’aumento della resistenza.
I DATI DELLA RESISTENZA AGLI ANTIBIOTICI, PROBLEMA ITALIANO E GLOBALE – Tra i punti di partenza del lavoro di SIMIT vi saranno i dati prodotti dalle diverse istituzioni internazionali che già hanno richiamato l’attenzione su questo fenomeno, che rappresenta la possibile causa di una prossima emergenza sanitaria internazionale. “I dati dell’OMS e dell’OCSE dimostrano che l’Italia è il primo Paese europeo per numero di infezioni e di morti, con circa 15mila decessi l’anno stimabili come causati da microrganismi resistenti agli antibiotici – evidenzia il Prof. Marco Falcone – Come indicato dai dati dell’Organizzazione Mondiale della Sanità, nel 2050 l’antibiotico-resistenza sarà la prima causa di morte a livello globale, provocando 10 milioni di decessi. Nel nostro lavoro verranno utilizzati anche i dati della rete ALARICO (Advancing knowLedge on Antimicrobial Resistant Infections Collaboration Network), da cui si evince che le infezioni da microrganismi multiresistenti carbapenetici, i più difficili da trattare, causano, rispetto ai microrganismi sensibili a questi antibiotici, un eccesso di mortalità che può arrivare fino al 20%”.
“Fino a poco tempo fa, questo fenomeno interessava solo marginalmente il Nord Europa, ma dati recenti dimostrano che i pazienti resistenti ai carbapenemici sono ormai epidemici in varie aree dell’Irlanda, del Regno Unito, dei Paesi scandinavi – aggiunge il Prof. Falcone – Il fenomeno della resistenza agli antibiotici, quale minaccia globale già identificata, si sta allargando rapidamente. Serve pertanto una risposta unitaria, di cui l’Italia può diventare capofila, in virtù, suo malgrado, della maggiore esperienza acquisita con l’elevata frequenza di queste infezioni. Il progetto Resistimit può dunque rappresentare uno slancio anche a livello internazionale”.
