Di dota di una convenzione per l’esercizio delle funzioni di integrazione socio-sanitaria Il documento è stato firmato dagli otto comuni del Valdarno aretino e dell’Azienda USL Toscana sud est Con riferimento alla Zona-distretto Valdarno, i Comuni di Bucine, Castelfranco Piandiscò, Cavriglia, Laterina Pergine Valdarno, Loro Ciuffenna, Montevarchi, San Giovanni Valdarno, Terranuova Bracciolini e l’Azienda Sanitaria Locale USL Toscana sud est, si sono dotati (con deliberazione n. 42 del 06/09/2022) di una convenzione per l’esercizio integrato delle funzioni socio-sanitarie in tutta la zona Valdarno.
I Comuni e l’Azienda USL Toscana sud est hanno avviato il percorso integrato della Non Autosufficienza e hanno iniziato a lavorare su quello relativo alla disabilità. La Convenzione – ha detto il Presidente della Conferenza dei Sindaci, Sergio Chienni – di cui anche la nostra Conferenza si è dotata è un atto dovuto come disciplinato dalla legge regionale n.40 del 24/02/2005. Il documento adottato dalla Zona delinea un percorso che inizia con la Non Autosufficienza, con l’obiettivo specifico di erogare servizi certi, sostenibili e modulati sullo stato di bisogno della persona. La finalità è quella di qualificare gli strumenti diretti a individuare il bisogno e i servizi per offrire risposte adeguate, formare gli operatori, implementare forme di collaborazione con soggetti e organizzazioni terze, pubbliche e private, al fine di favorire percorsi che privilegino la domiciliarità”. La Convenzione – elaborata secondo lo “Schema-tipo della convenzione per l’esercizio delle funzioni di integrazione socio-sanitaria” approvato con Delibera della Giunta regionale n. 886 del 30 agosto 2021 n.66 – ha validità quinquennale e potrà essere rinnovata e/o modificata con adozione di un apposito atto deliberativo della Conferenza dei Sindaci Integrata della Zona Valdarno. “La firma della convenzione tra Comuni e Asl che delinea un percorso integrato per quanto riguarda la Non Autosufficienza – ha commentato la Direttrice della Zona Distretto Valdarno, Stefania Magi – è un importante punto di partenza per ribadire la volontà di un territorio e di una comunità di assicurare servizi omogenei e integrati. Tutto questo è stato frutto di un lavoro che ha visto Asl e Comuni strettamente connessi con l’unico obiettivo di assicurare percorsi e soluzioni adeguati ai cittadini del Valdarno in base alle loro esigenze e ai loro bisogni di cura e assistenza a prescindere dal comune di residenza”.La convenzione definisce, in particolare, le modalità organizzative adottate in riferimento ai processi di programmazione e di partecipazione, all’integrazione sociosanitaria, al coordinamento interprofessionale, ai percorsi assistenziali integrati e alla realizzazione di servizi sociosanitari e sociali integrati. Con l’adozione della convenzione i Comuni della Zona Distretto Valdarno consentono la piena integrazione delle attività sociosanitarie evitando così duplicazioni di interventi tra gli enti associati; assicurano il governo dei servizi sociosanitari e le soluzioni organizzative adeguate per garantire la presa in carico integrata del bisogno sanitario e sociale e la continuità del percorso diagnostico, terapeutico e assistenziale; rendono la programmazione delle attività sociosanitarie coerente con i bisogni di salute della popolazione e promuovono l’innovazione organizzativa, tecnica e gestionale nel settore dei servizi sociosanitari di zona distretto.
