La tecnica portata avanti da oltre due anni negli ospedali di Arezzo e Grosseto permette di scoprire eventuali recidive e individuare la terapia più idonea grazie ad un prelievo di sangue Una tecnica innovativa che attraverso un semplice prelievo di sangue, in modo rapido e sicuro, permette di effettuare il follow up nei pazienti operati di tumore o sottoposti a terapia.
Si chiama Biopsia Liquida e in Asl Toscana Sudest è partita in via sperimentale oltre due anni fa, a fine 2020, negli ospedali di Arezzo e di Grosseto. «Una tecnica innovativa che consente attraverso un prelievo ematico di cercare gli elementi molecolari che indicano l’eventuale persistenza del tumore – spiega il dr Agostino Ognibene direttore del Dipartimento Medicina di laboratorio e trasfusionale -. È un protocollo che va avanti da oltre due anni e che applichiamo nei casi di tumore alla mammella, polmone, intestino, vie urinarie, melanoma, carcinoma gastrico, tumore epatico e alle vie biliari»
«Su queste linee tumorali abbiamo in corso una sperimentazione – prosegue Ognibene – soprattutto per quanto riguarda il polmone, l’intestino e la mammella, dove stiamo cercando di usare la biopsia liquida come strumento di monitoraggio della malattia residua, in parallelo con la diagnostica per immagini come la Tac o la risonanza». «Questo studio con il quale ad oggi abbiamo arruolato più di 60 pazienti – continua Ognibene – ci permette di seguirli e capire se possiamo anticipare la scoperta di eventuali recidive o di metastasi o la persistenza della malattia, prima dei tempi di rilevazione con la diagnostica per immagini. Uno studio i cui risultati preliminari saranno pubblicati su una rivista internazionale del settore e che ci fa intravedere risultati predittivi e promettenti». «La biopsia liquida – continua Ognibene – viene usata quando non abbiamo più il tessuto da esplorare e quindi grazie ad essa si ricerca la presenza di eventuali cellule tumorali e DNA tumorale circolante per quantificarlo permettendoci di monitorare molti tumori e, grazie allo studio del dna delle cellule fornire al clinico oncologo informazioni utili a monitorare la malattia o eventuali recidive, agire con terapie personalizzate, la target therapy, e ridurre in maniera importante il ricorso a chemioterapia e radioterapia».
«A differenza del monitoraggio effettuato con la diagnostica per immagini ogni 6 mesi – prosegue Ognibene – la biopsia liquida richiede il prelievo di sangue una volta al mese e ci permette di capire se c’è una recidiva prima ancora di individuarla con la tac». Quali vantaggi comporta questa procedura? «Un fortissimo vantaggio economico che si affianca alla tempestività nell’anticipare la scoperta di una eventuale recidiva e quindi migliorare la terapia. Abbiamo uno strumento in più per combattere il cancro. Inoltre il paziente è meno stressato anche se deve venire ogni mese a fare prelievo ematico».
La strumentazione necessaria è stata acquistata con fondi Calcit e fondi aziendali. «Non dobbiamo dimenticare – conclude Ognibene – i professionisti impiegati in questa attività scientifica: a Grosseto Maja Rossi e Sara Bardi e ad Arezzo Alessandro Pancrazi, Alice Moncada e Francesca Pompili che insieme hanno dato un grosso contributo all’implementazione di questa nuova tecnica».
