SENZA CARDIOCHIRURGIA, EFFETTUATO ALL’OSPEDALE DI FORLI’ Nei giorni scorsi è stato effettuato, nell’Emodinamica dell’Ospedale di Forlì il primo intervento di impianto di protesi valvolare aortica percutanea (TAVI) nell’ambito di uno studio, denominato TAVI at HOME, che valuterà l’efficacia e la sicurezza di questa tecnica, effettuata senza la necessità della cardiochirurgia in sede, in pazienti il cui rischio operatorio è stato giudicato proibitivo da un team multidisciplinare e quindi non trattabili per via convenzionale chirurgica.
L’intervento è stato effettuato con successo dalla équipe della AUSL Romagna composta dal dott.Fabio Tarantino, responsabile della Unità di Emodinamica di Forlì-Cesena, coadiuvato dal dott.Andrea Santarelli, direttore facente funzioni della Cardiologia e responsabile dell’Emodinamica di Rimini e dal dott. Marco Balducelli, responsabile dell’ Emodinamica di Ravenna. “La stenosi valvolare aortica è una patologia cardiovascolare estremamente grave che, se non trattata adeguatamente, ha una elevata mortalità ad un anno, addirittura superiore ad alcune forme di neoplasia – afferma il prof. Marcello Galvani, direttore della Cardiologia e responsabile del programma aziendale della AUSL Romagna di cardiologia interventistica – Purtroppo molti pazienti non possono essere sottoposti ad un intervento chirurgico tradizionale di sostituzione valvolare per via dell’elevato rischio operatorio”. Poprio per questo, negli ultimi dieci anni, si è affermata la “TAVI”, tecnica che consiste nell’impianto di una valvola biologica attraverso le arterie senza necessità di circolazione extra-corporea. Tale metodica è diventata di prima scelta in molti pazienti affetti da stenosi valvolare aortica, consentendo di curare, con ottimi risultati, anche coloro che fino a pochi anni fa erano esclusi da ogni possibilità di trattamento efficace. “Lo studio che stiamo conducendo – continua il prof. Galvani – vuole aprire una strada diretta per ampliare ulteriormente la possibilità di trattamento di questi pazienti, attualmente limitato dalla presenza della cardiochirurgia nella sede in cui viene eseguita la procedura. A fronte del crescente numero di pazienti che necessitano della TAVI i tempi di attesa sono sempre più lunghi. La possibilità di utilizzare, in Romagna, una rete clinica solida e collaudata, in sintonia con il centro di cardiochirurgia di riferimento (Maria Cecilia Hospital – MCH, attualmente diretta dal Prof. Carlo Savini) ci offre l’opportunità di effettuare questi interventi in sicurezza e nella massima collaborazione con i colleghi cardiochirurghi, migliorando il disagio per il paziente, non più costretto a trasferimento tra ospedali e riducendo le liste di attesa e i costi complessivi della procedura.” “L’intervento di TAVI consiste nel posizionamento di una protesi valvolare biologica attraverso le arterie femorali – precisa il dott. Fabio Tarantino – Dal mese di luglio 2019, come équipe di Ausl Romagna abbiamo effettuato presso Maria Cecilia Hospital 562 interventi con risultati eccellenti, frutto di un lavoro di squadra e multidisciplinare che coinvolge attivamente i colleghi anestesisti della nostra AUSL ed i colleghi cardiochirurghi di MCH, che ci supportano attivamente.Nella Cardiologia dell’Ospedale di Forlì – conclude il prof. Galvani – venne eseguita nel 1973 la prima coronarografia, indagine diagnostica che ha cambiato in modo significativo la diagnosi e la cura delle malattie cardiovascolari. All’epoca rappresentò per la nostra comunità un evento straordinario. Cinquant’anni dopo, la prima TAVI in Emodinamica a Forlì apre ancora una volta una strada e nuove possibilità terapeutiche.”
