Nella Asl Toscana sud est il servizio è stato istituito nel 2021, diventando un modello consolidato di assistenza integrata e di prossimità Più di 103mila accessi, 5630 persone assistite al domicilio e 364mila prestazioni erogate dal 1° gennaio al 29 luglio 2024 in tutta l’area provinciale Aretina da parte delle infermiere e degli infermieri di famiglia e comunità. Sono i numeri del modello organizzativo di assistenza di prossimità voluto dalla Direzione aziendale e messo in atto dal Dipartimento delle professioni infermieristiche e ostetriche dal 1° settembre 2021.

L’attivazione è stata preceduta da una specifica suddivisione territoriale per garantire prossimità di intervento e da un corso formativo per potenziare le competenze sulle cure territoriali e trova concreta attuazione su tutto il territorio dell’Asl Toscana sud est.Gli obiettivi del modello dell’infermieristica di famiglia e comunità mirano a rafforzare il sistema assistenziale sul territorio e a domicilio, per una presa in cura concreta delle persone con problematiche legate soprattutto alle cronicità e fragilità, promuovendo una forte integrazione tra i professionisti coinvolti nel processo di cura e privilegiando “la casa come primo luogo di cura”. Grazie ad una piattaforma di teleconsulto e teleassistenza è possibile contattare il medico di medicina generale, il medico specialista di riferimento e l’infermiere specialista delle varie discipline in modalità interattiva direttamente da casa, grazie alla dotazione di tablet, senza far spostare la persona fragile dal proprio domicilio. Il servizio è garantito tutti i giorni dalle ore 7 alle 20, compresi i festivi ed è svolto in collaborazione con le altre figure professionali. L’attivazione iniziale avviene attraverso il contatto telefonico con il Distretto di appartenenza territoriale. “A quasi tre anni dall’ istituzione dell’infermiere di famiglia e comunità nella nostra Azienda – sottolinea la direttrice del Dipartimento delle professioni infermieristiche e ostetriche, Vianella Agostinelli – sono stati raggiunti risultati importanti nell’assistenza integrata e domiciliare e nella risposta capillare del territorio ai bisogni di salute delle persone con cronicità e fragilità. La nostra rete assistenziale prevede la possibilità di attivazione di infermieri specialisti, certificati con master universitari post laurea, per prese in cura particolarmente complesse legate a problematiche respiratorie, lesioni cutanee difficili, assistenza alla nutrizione in situazione particolari, posizionamento accessi venosi, cure palliative e molto altro”.–
