Lo screening per i tumori cutanei non ha un razionale scientifico. Infatti il big killer della cute ovvero il melanoma ha una sopravvivenza > 90% e l’aumento di incidenza del melanoma cutaneo, entro il 3° posto in Italia < 50 anni, è collegata alla diagnosi precoce (> 70% in Emilia Romagna). Infine ha messo in evidenza lo sviluppo degli innovativi progetti di telemedicina del Programma “Melanoma Multimedia Education” per gli operatori sanitari e le piattaforme di teledidattica “Sole per amico for Kids e for Young” per la prevenzione nelle scuole, già attivate a Ravenna e “apripista” nel panorama regionale e nazionale
La promozione di corretti stili di vita, parte integrante del Codice Europeo Contro il Cancro, è stata sottolineata da tutti gli specialisti che hanno ricordato come la corretta alimentazione, l’aumento dell’attività fisica, l’abolizione del fumo, la vaccinazione da HPV ed evitare una eccessiva esposizione alle radiazioni solari siano obiettivi da perseguire e da raccomandare.
Ha chiuso il convegno la Dr.ssa Amelia Tienghi in quanto promotrice dell’evento.
L’adesione ai programmi di screening nella popolazione ravennate è tra le più alte in regione Emilia-Romagna e quindi a livello nazionale. Tuttavia, per quanto riguarda lo screening del colon-retto, pur essendo Ravenna la seconda provincia emiliano-romagnola come tasso di adesione ed in generale la nostra regione una delle più virtuose d’Italia, rimane ancora da “convincere” circa il 50% dei nostri concittadini che la ricerca del sangue occulto salva la vita! mIn questo screening le persone dai 50 ai 69 anni vengono invitate ogni 2 anni, con una lettera personale, a sottoporsi ad un test per la ricerca del sangue nelle feci eseguibile comodamente a casa. I tumori intestinali tendono a sanguinare ed il test è in grado di dimostrare anche piccole concentrazioni di sangue nelle feci. mLe persone che hanno sangue nelle feci, cioè le persone positive al test, sono invitate a eseguire un esame diretto dell’intestino, previo colloquio con un gastroenterologo, la colonscopia, che potrà confermare o meno l’esistenza di una lesione polipoide o di un tumore in fase precoce oppure concludere che il sanguinamento dipendeva da cause diverse. La colonscopia può essere effettuata in sedazione e con una preparazione più semplice rispetto al passato. E’ stato dimostrato che la riduzione dell’incidenza, quindi la diagnosi di nuove neoplasie, per chi ha partecipato allo screening è stata del 35% tra gli uomini e del 25% tra le donne e la riduzione della mortalità è stata del 68% per gli uomini e del 60% per le donne Proprio per gli effetti assolutamente positivi riscontrati in questi anni, la regione ha recentemente stabilito di estendere gradualmente, a partire dal 2025, fino a 74 anni (dai precedenti 69) la possibilità di partecipare allo screening del tumore del colon retto, uno dei più frequenti sia negli uomini che nelle donne”. L’adesione allo screening mammografico risulta nella nostra provincia circa dell’80%, tra le migliori in regione e quindi in Italia. E’ stato dimostrato che nelle donne aderenti a questo screening la riduzione di mortalità per tumore della mammella è stata del 54% e, secondo dati recenti del Registro regionale tumori, la sopravvivenza a 5 anni nella nostra regione ha raggiunto il 92%. Questi risultati sono stati raggiunti grazie allo sforzo organizzativo del Centro Screening e al continuo monitoraggio della qualità in tutte le sue fasi. Se necessario vengono utilizzati esami di approfondimento complessi come la microbiopsia su guida ecografica o mammografica (sterotassica o tomoguidata) e la Risonanza magnetica (in collaborazione con i Servizi di Radiodiagnostica di Ravenna Lugo e Faenza). mEssendo il CPO inoltre sede della Breast Unit, tutti i casi di sospetta neoplasia mammaria vengono seguiti nel percorso di diagnosi e cura da infermiere case-manager dedicate e, quando inviati a trattamento, vengono discussi dal team multidisciplinarecostituito da specialisti di tutte le branche interessate (radiologi, oncologi, chirurghi, anatomo-patologi, radioterapisti…) che prendono in carico la paziente dall’inizio alla conclusione delle terapie e anche rispetto a tutti i follow up. Complessivamente nell’anno 2023 sono stati oltre 17.000 gli accessi al servizio per la senologia, mentre sono 493 le neoplasie mammarie diagnosticate (486 donne e 7 uomini). Anche l’adesione allo screening del collo dell’utero risulta ottimale in provincia ed è aumentata dopo l’introduzione del test HPV. Aderire allo screening del tumore della cervice uterina ha portato ad una riduzione del 40% di nuovi tumori e del 50% della mortalità per questa neoplasia. Il test HPV attualmente viene proposto alle donne dai 30 ai 64 anni ogni 5 anni mentre alle donne dai 25 ai 29 anni viene proposto il Pap-test. Le donne che risultano correttamente vaccinate ovvero con almeno due dosi prima dei 15 anni di età, vengono invitate direttamente a 30 anni per eseguire l’HPV-test. In Romagna la copertura vaccinale negli adolescenti è del 75% per le ragazze, 71% per i ragazzi che risulta un buon livello rispetto al dato nazionale del 53%.
