Si tratta di uno strumento diagnostico che utilizza gli ultrasuoni per analizzare la rigidità o elasticità del fegato, che è un marcatore di fibrosi epatica, e quantificare la steatosi, cioè il fegato grasso.
«Più c’è fibrosi nel fegato più alto sarà il valore di rigidità e maggiore sarà il danno epatico, e più steatosi c’è nel fegato (cioè più grasso è il fegato) più alto sarà il valore che rileverà la macchina quantificando il grasso presente all’interno dell’organo – spiega la dr.ssa Laura Emma Lapini medico infettivologo e responsabile UOS Ecografia Malattie Infettive San Donato Arezzo -. È uno strumento importante che, insieme all’ecografia tradizionale, permette di effettuare valutazioni più approfondite nel fegato, in modo non invasivo. Se infatti l’ecografia ne permette uno studio morfologico, il Fibroscan documenta e quantifica la fibrosi e la steatosi. Questo tipo di studio del tessuto epatico, prima dell’avvento di questo tipo di macchinari, veniva fatto con la biopsia che adesso è, invece, utilizzata soltanto in casi limitati». Il Fibroscan è indicato nello studio e monitoraggio delle epatiti croniche virali (da virus B e C), autoimmuni, da abuso di alcool, nella cirrosi per valutare il rischio di complicanze, nei pazienti con persistente rialzo delle transaminasi e/o gammaGT, nel monitoraggio del fegato in pazienti che assumono farmaci che, nel tempo, possono arrecare danno epatico ed infine nella steatosi epatica (fegato grasso). L’esame viene eseguito dall’equipe medica della UOC Malattie infettive nell’ambulatorio ecografico del reparto. «La steatosi epatica, frequentemente associata a sovrappeso, obesità, diabete, dislipidemia – aggiunge la dr.ssa Lapini – è un quadro sempre più frequente e purtroppo molto spesso sottovalutato. Non tutti sanno, infatti, che nel 25% circa dei casi, nel tempo, la steatosi medio-severa può avere una evoluzione fibrotica fino ad arrivare alla cirrosi epatica e al tumore al fegato. Per questo è importante, una volta diagnosticata e determinato lo stadio di sviluppo della steatosi epatica, consigliare al paziente una correzione del proprio stile di vita attraverso una dieta adeguata, attività fisica e limitazione di bevande alcoliche, ricontrollando poi il quadro epatico a distanza di tempo anche grazie all’impiego del Fibroscan». «Ringrazio il Calcit e gli imprenditori che hanno scelto di donare questo importante apparecchio al reparto del dr.
